Intake & Referrals
This section provides information about: the programs and services at Across Boundaries and other mental health resources in the community, assessment of referrals, consultation, advocacy and referral to other services as needed.
Referral Sources
Referrals are accepted from the following sources:
- Individuals who have a serious mental illness/severe mental health problems
- Family members
- Health professionals
- Social service agencies
- Diversion programs
- Church, community organizations
Referral Procedures
A referral form must be completed by:
Referring person OR
prospective client
The referral form maybe completed by the intake worker over the phone.
Processing Of Referrals
- Upon receipt of referral, the intake worker will determine the appropriateness of the referral.
- An assessment interview will be arranged preferably at Across Boundaries. In some cases the assessment maybe done at home by a case manager.
- Referrals will be assigned to the waiting list and will be informed once a vacancy occurs. This process will be applicable for referrals that prefer the Case Management service at Across Boundaries only.
- In cases where there is an immediate need for a case management service or other needs that Across Boundaries is unable to provide, every effort will be made by the intake worker to assist the referral in accessing the most appropriate agency/service.
Referrals of Families
- Referrals from the same family will be counted as separate individuals.
- Family members who specifically ask for same case manager will be accommodated when possible and appropriate.
- Services will be provided to the client and if necessary also to the family.
- In cases where the family needs assistance for ongoing support, a referral maybe made to a family agency/organization.
